Click here to access Indiana’s petition form. After you open Indiana’s petition form, print a copy and then go out and collect some signatures! The Indiana form is two (2) pages - print it double-sided if possible. The second page is mostly for use by the County Supervisor of Elections offices. However, the "Affidavit of Assistance Provided to Petitioner(s)" section is important if you collect a signature from an individual that needs assistance with signing. Then you, as the assister, would fill in this section. To maximize the number of signatures collected, we recommend starting with friends and family. Then take a clipboard and your petition form to a busy location in your community - one great option is a farmers’ market. When collecting signatures, make sure you encourage people to write legibly. The cleaner our petition forms are, the higher the likelihood of signatures being accepted. Signatures must be collected on a form for the COUNTY that the signer is registered / eligible to vote in. If the signer is from another County, take a blank form and fill in the signer’s County in the top right and have them sign the new form.
